Accountable Care Organization (ACO)
Current Challenge
Trouble Balancing Costs with Savings
Agrees to be accountable for the quality, cost and overall care of enrolled beneficiaries. High Need High Cost (HNHC) patients who have three or more chronic diseases and functional limitation in their ability to care for themselves. ACOs have a strong base of primary care physicians, but unable to provide cohesive care to HNHC patients.
The absence of an entity focused to coordinate HNHC care prevents the ACO to achieve their savings goal.
