Physician led Embedded Care
We are experts in complex care and operate as a hub for prompt information dissemination on behalf of the patient. We perform in-home assessment and patient education along with timely interventions led by experienced physicians. We emphasize a clear communication and value the patient and family involvement.
Our services include, post discharge followup, 24/7 nurse on call, disease specific program customized to an individual patient, transitional care intervention, PCP/Specialist coordination and any other interventions appropriate to the patient's condition.
